Continuous Glucose Monitoring Program Support

Feature Overview

Impact programs can now be set up with a “Continuous Glucose” (CGM) measurement type. CGM programs allow clinicians to track CGM use through providing summary statistics and the Ambulatory Glucose Profile (AGP) report in a non-real time workflow.

  • Sources Supported:

    • Abbott (requires separate agreement with the vendor)

    • Dexcom (requires separate agreement with the vendor)

  • Data Availability:

    • Summary statistics for most recent two week period

    • AGP report (most recent two weeks)

    • Data is pulled daily for the prior two weeks (not a real time integration)

  • EHR Support:

    • The AGP report can be sent as a clinician document to the EHR via a manual push from the clinician dashboard.

Impact Admin

Configure the CGM program via Impact Admin.

Note that your account must be enabled for CGM support. Please contact your Validic representative for more information.

 

  1. Navigate to Impact Admin

  2. Click “+ Create Program”

  3. Enter program name, description, select organization

  4. Select the “Continuous glucose” measurement type for the program. Note that continuous glucose can also be combined with other measurement types (for example a CGM and weight program).

  5. Click “Save” - the program is now available for enrollment

  6. Configure other program settings as desired

  7. Program Goals and Events/Actions are not yet supported for CGM

  8. Programs can be configured with either web enrollment (where the patient is provided a link in the enrollment email to complete a web enrollment workflow) or HealthBridge enrollment (pending release)

Impact Clinician

Enrollment and viewing of the CGM data (summary statistics and AGP report) from the Clinician Dashboard. Program/Panel view available for at a glance time-in-range for most recent two weeks of CGM data.

  1. Enroll participant into the newly created CGM program. Note that Goals and Events/Actions are not currently supported for CGM.

  2. Participant completes enrollment.

  3. Once the participant connects their CGM vendor account (Device Account Synced status), data flow can be initialized. Data is pulled daily for the last two weeks of CGM information. The data pull happens at a random time daily.

  4. After the first data pull, the participant will transition into Initial Data Uploaded status and summary statistics and an AGP report will be viewable from the Impact Clinician Dashboard.

    1. A note on the dashboard view window: the data context viewable will be the most recent two weeks of data for any given view window.

  5. AGP Report information

    1. For EHR integrated customers, the AGP report (pdf) can be pushed as a clinical document to the patient chart via a button on the dashboard.

    2. For non-EHR integrated customers, the AGP report (pdf) can be downloaded via a button on the dashboard.

 

Program/Panel View

The program/panel view display the most recent two weeks of CGM time-in-range information for program participants.

Participant Workflow

Participant workflow overview. Enrolling into the program and connecting the CGM account to initiate data flow.

 

Two participant workflows are supported:

  • Web enrollment

  • HealthBridge enrollment

 

The Abbott CGM integration requires that the user has an Abbott account. The user must permission their CGM data to you via the Abbott web permissioning process to complete enrollment and initiate data flow.

Web Enrollment

  1. Once enrolled, the participant will receive an email with a link to the web enrollment process.

  2. Participant clicks the link in the enrollment email. Note that the content in this email can be customized within the program in Impact Admin.

  3. Participant enters their last name and phone number. Clicks “VERIFY IDENTITY”

  4. Participant consents to participate in program via clicking “AGREE AND CONTINUE”

  5. Participant clicks “Abbott - Securely Share Data” button. “Dexcom - Securely Share Data” will also show if enabled for your account.

  6. Participant enters source credentials used to login to source/vendor app and clicks “SIGN IN”

  7. Participant selects scopes of data permissions and clicks “ALLOW” to initiate data sharing.

  8. Enrollment complete - the participant is now in Device Account Synced status and will transition to Initial Data Uploaded on the first pull of CGM data.

  9. Once the data pull has completed, CGM data will be available in the clinician dashboard for the participant.
