Continuous Glucose Monitoring (CGM) Support for Impact Programs

CGM programs can now be enabled within Impact. Impact provides support for both Abbott and Dexcom sources and allows the viewing of summary statistics and the Ambulatory Glucose Profile (AGP) report for the most recent two weeks for the participant from within the Impact Clinician Dashboard. AGP report delivery to EHR and EHR discrete data support for summary CGM data is also available.

Features:

Viewing of summary CGM statistics and AGP report within Impact Clinician Dashboard
Support for both Abbott and Dexcom CGM sources
AGP report delivery to EHR as pdf document (Epic and Oracle Health)
Discrete data support for writing summary statistics to the EHR (Epic and Oracle Health)
Program/Panel view of CGM program participants by time in range
Program set up remains modular, where additional supported measurement types can be included in the same program with CGM (BGM, weight, blood pressure, SpO2, body temperature)
HealthBridge mobile app enrollment, source connection, and summary data viewing (browser-based participant enrollment experience also supported)

*Please note that this is not a real-time integration - two weeks of summary data is pulled daily.
