Participant: HealthBridge Express Enrollment and Access

Once the participant has been enrolled in a HealthBridge Express Care Kit program (see: Impact Clinician: Enrolling a Participant in a Remote Care Program: HealthBridge Express (Kit/Fulfillment Workflow) ), the fulfillment partner will pre-assemble and provision the care kit for the participant. HealthBridge Express will be installed and configured on the hub (tablet or phone), the requisite peripheral devices will be paired, and the kit will be packaged and shipped to the participant. The participant can then access the HealthBridge app on the hub.

 

1. Access HealthBridge Express

Select the “HealthBridge” app on the hub or the app will auto-launch (depending on configuration) to a PIN login screen.

 

2. Enter PIN to authenticate with the HealthBridge Express app

The participant will enter their 4-digit PIN configured during clinician enrollment to access the app.

Note: if the participant has forgotten their PIN, it can be reset from the Clinician Dashboard (Impact Clinician: Resetting PIN for Participant ).

3. Click “I Agree” to consent to the program

On first accessing the program, the participant will see a program consent screen. The participant must review the consent content and click the “I agree” button to continue to the Dashboard.

4. Begin taking readings with the devices

The participant will then be taken to a program Dashboard screen. New readings will show here when available.
