This article pertains to: IMPACT

Once you (the clinical user) have enrolled a participant into a remote management program, and the participant has completed enrollment into the program, data flow for that participant will initiate, and you will begin to receive participant readings into the Impact Clinician Dashboard. You will also receive any associated alerts or notifications for the participant.

The Clinician Dashboard is organized into two primary areas:

  1. Participant information section - this section contains information specific to the participant that is independent of a program.

  2. Program information section - this section contains program specific information. If the participant is in enrolled in multiple programs, those programs can be navigated between using the program tabs.

There are a number of features in Impact that can be turned on or off in Impact based on a clients needs and how they are accessing the software. There is a chance there may be elements of the product discussed below that don't display in your instance of Impact. What you see in Impact would have been a decision made by your administrators during implementation of Impact in your environment. We're offering information on all available features in Impact below to support all of our clients with one piece of documentation.


1. Participant Section (upper section)

This section contains top level information for the participant - this includes the patient’s demographic information, alerts across all programs, and notes and alert dismissals across all programs. Note that your organization may not be using all of these features.

Participant Demographics and Editing

  • Participant information shows across the top of the view - this includes: participant name, identifier (if enabled for your organization), DOB, mobile phone number, and email address.

  • Click the pencil icon to update the participant's information, for example, their email address or mobile phone number.

Alerts

  • Alerts will trigger based on their configured criteria and will accumulate in the alert box - this is similar to an inbox for new messages or notifications, but it is contained within Impact.

    • Priority alerts are shown with a red “!” icon.

  • Only active alerts will show in the alert box. Alerts can be selected and dismissed, and notes and time (duration of time that you spent with the participant around the alert, if any) can be logged.

  • Once an alert has been dismissed, it will disappear from the alerts box and be logged as a dismissal.

Alert Dismissals / Notes

  • Notes can be added when dismissing an alert or independently via the ADD NOTE button, as needed.

    • The note itself and the time spent can be edited after the note has been created by the original author. Notes can also be deleted by the author.

    • You can enter time (duration of time that you spent with the participant) as a component of the note.

Time Spent

  • Time spent in minutes can be logged to track interactions with participants.

    • You can enter time (the duration of time [mins] that you spent with the participant) while dismissing an alert or when adding or editing a note.

    • Time is summed at the participant level for the current month and then presented in the Participant View. You can also select the month to be able to review total time from past months (up to six months in the past). This time is a sum of time logged for the participant, across programs, and across providers (if multiple providers have logged time with the participant).


2. Program Section (lower section)

This section contains program level information with data and information specific to the selected program.

Switching Between Programs

You can switch between programs by using the program tabs. Select the desired program tab to navigate to that program. Note that programs that are pending enrollment will prompt for you to complete the enrollment.

+ Enroll in New Program (enrolling participant in another program)

Participants can be enrolled in multiple programs. To enroll the participant into another program, click the + ENROLL IN NEW PROGRAM button to initiate the enrollment workflow.

Note: Once enrollment has started, or the participant has been enrolled in the new program, the program will show as a program tab option.

Program Status Indicator

The enrollment status is shown at the left hand side of the dashboard. With this, you can understand how far along the participant is within enrollment for the selected program.

Data Timespan (View Window)

The Data Timespan is shown on the right hand side of the dashboard. This is extremely important, as it informs the date range for the entire section below, including the logbook, graphs, statistics, and goals.

You can toggle the window by clicking on it. Two weeks is the default view, and there are four total window options:

  1. One week

  2. Two weeks < Default View

  3. One month

  4. Three months 

Program Actions

Select the Program Actions ellipses button (...) within the dashboard.

Additional program actions and options will be shown for that participant based on the participant’s program status Impact Participant Program Enrollment Statuses.

Options and information include: enrolling and reviewing provider information, an option to resend the enrollment email, an option to change reviewing provider, as well as an un-enrollment button if the participant is active in the program.


Resending Enrollment Email

The Program Actions modal will show the Enrolling and Reviewing Provider for the selected program. If the participant has not completed enrollment (Impact Participant Program Enrollment Statuses), the clinical user will have the option to RESEND THE ENROLLMENT EMAIL to the member.

The participant will then be send another copy of the enrollment email to prompt them to complete enrollment (Participant Enrollment Workflow Overview: HealthBridge ).

Changing Reviewing Provider

The Program Actions modal will show the Enrolling and Reviewing Provider for the selected participant’s program. If you are not the Reviewing Provider, you can take over and assume that role for the patient.

The Reviewing Provider will always take on the required notifications (Events/Actions) for the participant. To take over as the Reviewing Provider, you will be prompted to confirm that you would like to proceed and assume the role. Once you have assumed the Review Provider role, all required notifications (Actions on Events) will be enabled for you.

Unenrolling the Participant from the Current Program

If the participant has completed a program, or if you need to un-enroll them from the current program, select the UNENROLL PARTICIPANT button from the Program Actions modal.

You will be prompted to confirm un-enrollment. Un-enrollment cannot be undone; once unenrolled, the participant’s data will stop flowing, and you will no longer receive notifications for the participant. Once unenrolled, the participant is eligible for re-enrollment in the program. Click the YES, UNENROLL button to complete un-enrollment.

Program Summary

The Program Summary section provides an at-a-glance view of measurement types used within the program, summarized statistics for those measurement types, and the participant’s source connection status (Impact Participant Source Connection Status) by measurement type.

Advanced Statistics (Blood Glucose programs only)

The Advanced Statics section can be expanded to show additional data and summary metrics for blood glucose programs. These include the number of hyper and hypoglycemic events, the average number of readings taken per day, and other more granular statistics.

This section relies on a specific goal to be configured (required and enabled) for the program in order to generate these statistics - “Keep all blood glucose between ___ and ___.”

Program Readings (participant data)

The program readings section contains the participant’s reading information and data. The participant’s data can be viewed in a traditional logbook view, or viewed in graphs and charts relevant to the program’s measurement types.

Logbook View

The logbook is the default view for participant data. Individual data points show within the logbook, and are organized by row (day) and column (time of day) for the blood glucose measurement type (these show as a hybrid calendar view for other measurement types). The logbook and other charts will show the data available only for the Data Timespan range selected.

Readings in Red ↑ signify high out of goal range values
Readings in Blue ↓ signify low out of goal range values

The logbook and other graphs can be filtered to show desired reading types. For example, pre and post-meal values for blood glucose.

Comment bubble icons next to the reading dates indicate that the participant has entered comments for readings during that day. The number above the icon shows the number of comments. The icon can be clicked, or the individual readings can be clicked, to expand and show the reading comments.

Reading Origin

Participants can submit readings in to Impact via a connected device or via manual entry (if enabled for the account). Manually entered readings will show with a manually entered reading icon.

  • Device entered - no icon

  • Manually entered - manually entered icon

If manually entered readings are present in the dataset for the selected view window, an information banner will also display notifying the clinician that manually entered readings are present.

Manually entered readings can only be submitted via the HealthBridge app and not via other supported data sources.

Logbook and chart views differ by measurement type.

Impact Measurement Types

Charts

Readings can also be displayed in graphical or chart views. Click the “View” dropdown on the top right of the section to select the “Charts.”

The Charts view shows a Timeline, as well as a Time of Day graph for the blood glucose measurement type. Different measurement types have different graphs relevant to those measurement types.

These graphs can be filtered by meal relation (for Blood Glucose) or to only show out of range values. It is important to again note that these graphs only show values within the Data Timespan selected.

Manually entered readings show in the reading detail view with an icon an “manually entered reading” label.

Goals

Goals show participant program progress at the current point in time (upon page load). These are also dependent on the Data Timespan selected. Individual participant goals can be edited and updated to set personalized care objectives - these changes will only apply to the individual participant for the specific program.

Default program goals are established by a Validic Admin via the Admin Portal.

Goals are shown to the participant within the HealthBridge app. Note that the participant does not see their goal status or their progress against the goal, only the goal description itself.

Events/Actions (Notifications)

Events, such as a high blood pressure reading, can queue Actions, like sending an EHR message to a Provider. Events/Actions can be set up for both participant and clinician users, and they are defined by an Impact Admin for each program.

Clinician

Providers can receive notification Actions via the EHR messaging platform (either direct or via a group or pool message), via in app Alerts, or via email or SMS (not widely used).

Participant

Participants can receive notification Actions via email, SMS, and patient portal/secure message (if enabled).

If you are the participant’s Reviewing Provider/Clinician, you will have required Actions enabled by default and those Actions will be sent or assigned to you. You can enable optional Actions via the toggle to the left of the Action to opt in to it.

If you are not the participant’s Reviewing Provider, you can still opt in to receiving notifications (Actions) for the participant as an Observing Provider/Clinician using the opt in toggle.

Events/Actions can be edited using the “Edit” button on the top right of the section. 

It is important to SAVE the changes once you are done editing.

Note that there is a single instance of each event per participant - any updates made to editable event values will apply to all recipients of the action for the event. A “last updated” name and timestamp will be applied after the event edit has been saved.
