The Additional Program Information panel will show the Enrolling and Reviewing Provider for the selected member’s program. If the member has not completed enrollment (Impact Member Program Enrollment Statuses), the clinical user will have the option to RESEND THE ENROLLMENT EMAIL to the member. The member can then use the link in the enrollment email to complete member enrollment to connect a data source and become active in the program (Member Enrollment Workflow Overview).

Advanced Statistics (Blood Glucose Programs Only)

The Advanced Statics section can be expanded to show summary information and additional data for blood glucose programs. This includes the number of hyper and hypoglycemic events, average number of readings taken per day, and other more granular statistics.

2. Program Summary

The Program Summary section provides an at-a-glance overview of the member’s program status, source connection status, and reading data.

On the right hand side, you can see the member's program status (Impact Member Program Enrollment Statuses). You can also see the member's data source connection status (Impact Member Source Connection Status). If there are more than one measurement types available in the program, the binary status (pending and connected) of each source can be seen.

3. Program Readings (Member Data)

The Program Readings section contains the member’s discrete data. The member’s data can be viewed in a traditional log book view, or viewed in graphs and charts relevant to the program’s measurement types.

Log Book View

The log book is the default view for member data. Individual data points show within the log book, and are organized by row (day) and column (time of day) for the blood glucose measurement type. The log book and other charts will show the data available for the View Window selected at the upper right hand corner of the clinician dashboard.

Readings in Red ↑ signify high out of goal range values
Readings in Blue ↓ signify low out of goal range values

The logbook and other graphs can be filtered to show desired reading types. For example, pre and post-meal values for blood glucose.

Comment bubble icons next to the dates on the far left column of the log book indicate that the member has entered comments for readings during that day. The number above the icon shows the number of comments. The icon can be clicked, or the individual readings can be clicked, to expand and show the reading comments.

Different measurement types have different log book and chart views - Impact Measurement Types.

Chart Views

Readings can also be displayed in graph views. Click the View dropdown on the top right of the section to select the Charts view.

The Charts view shows a Timeline, as well as a Time of Day graph for the blood glucose measurement type. Different measurement types have different graphs relevant to those measurement types. We are looking at blood glucose data via the Timeline and Time of Day graphs below. These graphs can be filtered by meal relation and to only show out of range values. It is important to again note that these graphs only show values within the View Window selected at the upper right hand portion of the clinician dashboard.

4. Program Configuration

Goals

Goals show at-a-glance member program progress. Individual member goals can be edited and updated to set personalized care plans and care objectives. Default program goals are established by a Validic Admin via the Admin Portal.

Notifications

Notifications are intended to alert necessary parties when certain rules are met. There are two available notification recipients - the member and the provider. Providers receive notifications via integrated messaging platforms - EHR or care management platform messaging (depending on how this is implemented). Members can receive notifications via email, SMS, and patient portal (if enabled).

If you are the member’s Reviewing Provider/Clinician, you will have required notifications enabled by default. You can enable optional notifications via the toggles to the left of the notifications.

If you are not the member’s Reviewing Provider, you can still opt in to receiving notifications for the member as an Observing Provider/Clinician.

Notifications can be edited using the “Edit” button on the top right of the section.

It is important to SAVE the changes once you are done editing.
