Participant: a participant is the user enrolled in the remote monitoring program. The participant may be referred to as a “patient” or “member” in your organization. Participants are enrolled into remote monitoring programs by clinical users.

1. (EHR Integrated) Navigate to Participant

in EHR and Launch Impact
Navigate to the participant that you wish to enroll in your EHR or , care management, or host platform (if available).On the screen you will see a list of available programs. The launch workflow (to launch Validic Impact) differs depending on your platform and implementation.
If the participant has been previously enrolled (or an enrollment has been started), you will land on the clinician dashboard for that participant. A new enrollment can be started or an existing enrollment can be completed from the dashboard.
If the participant is already enrolled in another program, click the program dropdown, and click the +ENROLL IN NEW PROGRAM button to begin enrollment into another program.

Select the program that you want the participant to participate in. Participants can be enrolled in multiple programs, but must be invited to each program individually.
Click the ENROLL PARTICIPANT button. If you select the wrong program, simply click the BACK button to select the correct program.

2. Update Participant Contact Information and Select Program
Update the participant’s contact information using the enrollment form.
The participant’s first name, last name, DOB, and identifier (this may be an MRN, FHIR ID, or other ID that you are using to uniquely identify the participant) will populate. This information is pulled from the EHR and is not editable for most implementations.
The participant’s email address and phone number are also populated from the EHR and can be updated during the enrollment.
A "preferred language" dropdown option will show if the program supports languages other than English. The participant’s preferred language can be set, and they will then
receive email and sms content in that language (if supported by the program). The preferred language will default to English.
2. Review Program Goals & Notifications
Review the program goals and notifications for the participant. Some program configurations allow the editing of goal and notification values. Select the EDIT button to modify goals and notifications at the participant level.

Select the desired program for enrollment.
The program list shows available programs, along with program name, description, and measurement types tracked.

Click the “Next” button to continue the enrollment.

If “identifier” is included within your workflow, you will need to enter this correctly initially, as there is not an option to update it once it is entered. |
3. Review Program Goals
Goals help to track and motivate participant progress within remote care programs. These are tracked via the Clinician Dashboard, and certain goals inform visualizations within the dashboard (examples: in and out of range, blood glucose advanced statistics, and chart goal overlays). Goals also show to the participant within the HealthBridge app. Goals are defined by an Impact Admin for each program.

Update goal thresholds if desired. Goals are specific to the participant and any changes to the individual participant’s goals only apply to that participant.
Click the “Next” button.
3. Review Program Events/Actions (Notifications)
Events, such as a high blood pressure reading, can queue Actions, like sending an EHR message to a Provider. Events/Actions can be set up for both participant and clinician users, and they are defined by an Impact Admin for each program.
Update Event thresholds or values, if desired. For example, adjust a blood glucose threshold for a diabetic participant or change a systolic blood pressure threshold for a participant with hypertension.
Review program Actions. Make any necessary adjustments to the Action settings, if available.
Required: these Actions are set as required by a Program Admin and can not be disabled for the reviewing provider (reviewer).
Enabled Toggle: use to enable/disable a notification. Note that for the reviewing provider, Required Actions do not have a toggle and cannot be disabled.
Frequency: the Action frequency can be updated as desired.
Events and the associated Action are specific to the participant and any changes to the individual participant’s Event or Action is only applicable to that participant.
Click the “Enroll” button to enroll the participant in the program and send the program invitation email. (if you see a “Next” button instead, please see the below notes):

Click the COMPLETE CLINICIAN ENROLLMENT button. An enrollment email is sent to the participant.

The participant has now been enrolled in the program, and their program status will transition to Enrolled by Clinician
