Note this page only applies to organizations enabled with and using fulfillment workflows via HealthBridge Express to deliver packaged “remote care kits” to participants.

1. Go to Validic Impact

Using your username and password, access Impact. If you do not recall this information, use the "forgot password" feature to reset your password. 

2. Click on ENROLL

3. Enter the Participant’s first and last name.

4. Collect email address and phone number if possible

While not required for HealthBridge Express, collecting this information is always helpful. The shipping carrier may wish to contact the participant to facilitate delivery in some cases.

5. Enter the date of birth

Be sure this information is accurate.

Note: For Providers enrolling participants inside an EHR, if the date of birth is incorrect, please abandon enrollment, correct the DOB in your EHR, and restart enrollment. Your EHR should be the source of truth for all demographic data to ensure data stays aligned.

6. Chose the program to enroll the Participant

7. Configure your Goals and Events/Actions (Notifications)

If needed, configure the program's Goals and Events/Actions. Once complete, select "Next" to advance.

8. Enter the shipping address

9. Enter Participant Access PIN

Ask the Participant to provide a four-digit PIN. The Participant will use this PIN to access HealthBridge, so please ensure it's a PIN they will remember.

Note: you can reset an existing participant’s PIN from the clinician dashboard.

10. Select Devices

Chose the size and type of device to send to the Participant. Ensure these values are correct. They cannot be edited once the Participant is enrolled in the program.

11. Click on NEXT

With devices selected, click next.

12. Validate Address

In the event the address is incorrect, Impact will present alternate addresses. Confirm all address changes with the Participant since this could delay delivery.

13. Click on ENROLL

With information entered and confirmed with the Participant, select "Enroll" to enroll the Participant.

14. Monitor

Accessing the Participants dashboard in Impact will show the status of enrollment. Once the Participant receives the kit and consents to participation, their status will update in Impact.
