Once you (the clinical user) have enrolled a member participant into a remote management program, and the member participant has completed enrollment into the program, data flow for that member participant will initiate, and you will begin to receive PGHD into the Impact Clinician Dashboard. You will also receive any associated notifications for the memberparticipant, if enabled.

Programs and View Window
Member Participant programs and the view window is shown in the header area of the dashboard. You can view the member’s participant’s name and the programs that they are enrolled in (shown as a program dropdown menu).

Members Participants can be enrolled in multiple programs, if available. Select the highlighted program in the header. A dropdown menu will display. Click the + ENROLL IN NEW PROGRAM button to enroll the member participant into another program.
Note: Once the member participant has been enrolled in the new program, that program will show in the programs dropdown in the clinician dashboard header.

Additional program information and options will be shown for that member participant based on the member's the participant’s program status Impact Member Participant Program Enrollment Statuses. Options and information include: enrolling and reviewing provider information, an option to resend the enrollment email or source invite email, an option to change reviewing provider, as well as an un-enrollment button if the member participant is active in the program.

The Additional Program Information panel will show the Enrolling and Reviewing Provider for the selected member’s participant’s program. If the member participant has not completed enrollment (Impact Member Participant Program Enrollment Statuses), the clinical user will have the option to RESEND THE ENROLLMENT EMAIL to the member. The member participant can then use the link in the enrollment email to complete member participant enrollment to connect a data source and become active in the program (Member Participant Enrollment Workflow Overview: Web Enrollment).
Resend Source Invite Email
The Additional Program Information panel will show the Enrolling and Reviewing Provider for the selected member’s participant’s program, as well as the date of enrollment. If a new measurement type has been added to the existing program (Adding a Measurement Type to an Existing Program), but the member participant has not yet connected the device, the clinician will have the option to RESEND THE SOURCE INVITE EMAIL to the memberparticipant. The member participant can then use the link in the email to complete the steps to sync a device for the new measurement type and enable data sharing.

The Additional Program Information panel will show the Enrolling and Reviewing Provider for the selected member’s participant’s program. If you are not the Reviewing Provider, you can take over and assume that role. The Reviewing Provider will always take on the required notifications for the memberparticipant. To take over as Reviewing Provider. You will be prompted to confirm that you would like to proceed and assume the role of Reviewing Provider. Once you have assumed the Review Provider role, all required notifications will be enabled for you.

If the member participant has completed a program, or if you need to unenroll them from the current program, select the UNENROLL MEMBERPARTICIPANT button.

You will be prompted to confirm un-enrollment. Un-enrollment cannot be undone; once unenrolled, the member’s participant’s data will stop flowing, and you will no longer receive notifications for the member participant (if enabled). Once unenrolled, the member participant is eligible for re-enrollment in the program. Click the YES, UNENROLL button to complete un-enrollment.

The Program Summary section provides an at-a-glance overview of the member’s participant’s program status, source connection status, and reading data.
On the right hand side, you can see the member's the participant’s program status (Impact Member Participant Program Enrollment Statuses). You can also see the member's the participant’s data source connection status (Impact Member Participant Source Connection Status). If there are more than one measurement types available in the program, the binary status (pending and connected) of each source can be seen.

Participant Data)
The Program Readings section contains the member’s participant’s discrete data. The member’s participant’s data can be viewed in a traditional log book view, or viewed in graphs and charts relevant to the program’s measurement types.

The log book is the default view for member participant data. Individual data points show within the log book, and are organized by row (day) and column (time of day) for the blood glucose measurement type. The log book and other charts will show the data available for the View Window selected at the upper right hand corner of the clinician dashboard.
Readings in Red ↑ signify high out of goal range values
Readings in Blue ↓ signify low out of goal range values

Comment bubble icons next to the dates on the far left column of the log book indicate that the member participant has entered comments for readings during that day. The number above the icon shows the number of comments. The icon can be clicked, or the individual readings can be clicked, to expand and show the reading comments.

Goals
Goals show at-a-glance member participant program progress. Individual member participant goals can be edited and updated to set personalized care plans and care objectives. Default program goals are established by a Validic Admin via the Admin Portal.

Notifications are intended to alert necessary parties when certain rules are met. There are two available notification recipients - the member participant and the provider. Providers receive notifications via integrated messaging platforms - EHR or care management platform messaging (depending on how this is implemented). Members Participants can receive notifications via email, SMS, and patient portal (if enabled).
If you are the member’s participant’s Reviewing Provider/Clinician, you will have required notifications enabled by default. You can enable optional notifications via the toggles to the left of the notifications.
If you are not the member’s participant’s Reviewing Provider, you can still opt in to receiving notifications for the member participant as an Observing Provider/Clinician.
